I would be surprised if you as a client need to have an account to access this service. I think that what is missing here is a URL in the Go to box on the second sheet. This will contain a unique name identifying the location of your appointment. Without this you can't try the appointment procedure out.
Possibly if you need to reply to the letter an accept the appointment they will send a new sheet with a Go to: URL filled in, to you?
It is vague and incomplete at this stage of the appointment booking process but hopefully this will be rectified assuming that there are further steps to take.
