Exactly what it says on the tin. Forgive me if this is a little rusty, this is the literal first time I've ever done something like this.
I was hoping to find a "Create Your Own Story" (or CYOS) thing on here somewhere, but after going through four pages of search results using the keywords "create your own story" and simply "CYOS", even limiting it down to just the "Forum Games" subforum, I found nothing relevant to what I wanted. Well, I guess if you want something, you've gotta do it yourself.
These things usually work something like this, I start off with a prompt: the summary/synopsis of the story, the character(s), the setting, etc. and then, post-by-post, the story is continued through the thread. However, I'm going to keep it vague to make it easier for you folks to come up with your own ideas along the way.
The only real rule I have is to differentiate what's a part of the story from what's meta somehow whenever you make a meta comment; you can colour it, italic/bold it, do whatever you want with it, I dunno, to keep anyone from getting confused. Well, that rule and the rules of the forum as a whole, of course.

You wake up in a hospital. As far as you can tell, you're completely alone there. You don't know why you're there, how you got there, and can only remember the simple things, such as your name and your birthday, thanks to the plastic bracelet you're wearing.
You seem to be unhurt, so you get up and leave the room finding that the hospital is, in fact, completely devoid of other people. No doctors or nurses, no other patients from what you can see, nothing. The power is out, the only light source the dimming sunlight basking in from the windows. The whole place appears to be in disarray, as if something bad happened there.
What do you do next?
