Short term memory issues at work

I currently seem to be experiencing frequent short-term memory issues at work due to my anxiety. I am forgetting what I have discussed with people, and am repeating questions or discussions that I have already had. I am forgetting what I have observed, and so find it difficult to take notes on what I have observed, even if it is only a few minutes before. I think it is just because I am anxious, as when I am at home & relaxed I can often remember things much better. My colleagues are starting to notice my memory issues & though I have joked it off so far - it is starting to concern me. Does anyone else have similar memory issues & if so how do you deal with it at work?
