Tony, have you ever thought through what it would actually be like to get arrested and go to jail for a few hours? What you described could get you arrested.

The thought of being arrested terrifies me. I’m worried about you.

there was Show & Tell? Kids bring in stuff from home (that's the 'show' part)
and talk about it (that's the Tell.)

(walking into traffic, tossing trash cans around etc.)

How about working on the the 'tell' angle?
By this I mean TALK TO YOUR THERAPIST.
Describe what you've been doing to try to get people to notice your misery.
Use your words. Sure, you could act it out, too.
But TALK TO YOUR THERAPIST.

What were you expecting? People to get out of their car and provide free on the spot therapy? Agree to go out on a date with you? Acting out in public sends a message that you're emotionally disturbed. People are likely staying away because they're afraid you'll harm them. If you want friends, go to a soup kitchen, a meetup group for the mentally ill, or volunteer at a nursing home. You have plenty of options.

There must be something they can give you, to keep on you in an emergency. Something that can sedate you. You're going to get run over. Or hurt seriously. Or someone might hurt you.

New York City law enforcement is the toughest, most sociopathic cops in the country. They'll pound your head into the concrete until it cracks open and your brain falls out. And then there's Rikers Island, easily the worst municipal jail in the US.

You won't have any legal representation except for an overworked law student who simply tells you what you're charged with and how to plead guilty to everything in the correct manner. Then you get sent to New York State's notorious prison system. Guys like you get killed in a few weeks time. New York City is not the place to act crazy in public.

Guess I’ll say now that my immediate reaction to seeing this last night was the urge to say “If you’ve now tried to engage with a car during an outburst, you’re on the verge of arrest.”

Honestly maybe it's best that you do get arrested. You don't seem to be able to monitor and control your own behavior and you walking out into traffic is putting other people, who are minding their own business, at risk.
