And the other things you repeat here, like
wishing you'd been aborted, and how
suicide seems to an option, how you feel
that you fail at everything.

Use the language you use here.

I tend to not have a “script” and I don’t know what to say most of the time.

Unless the statements and poignant musings here are exaggeration for effect
and the bland watered down things you do say in the doctor's office
more aptly describe your situation.

PS: It's OK to give a doctor a WRITTEN statement.
